Please arrive approximately fifteen minutes prior to your scheduled appointment.

Please download and print the new patient packet have it completed prior to arriving at our office.

Please have your most recent imaging on CD when you present for your appointment.  We read all films ourselves which is more important than the reports by the radiologists.

Although we attempt to stay on time, we cannot predict how much time an appointment will require. We give every patient the time that they need to be evaluated, educated, and given a plan of diagnosis or treatment. This can cause us to be behind schedule. We appreciate your consideration and understanding that chronic pain can be very challenging to successfully treat and every patient is equally important.
